The T2G National Day of Action is quickly approaching and our Plank Mentor videos are now available!

Last year for our National Day of Action, we introduced the concept of the T2G Talk & Taste. It was such a success that we are bringing it back again this year! On Nov. 7, we’re encouraging you to assemble your diabetes team for another T2G Talk & Taste. This year, we ask that you watch a brief video Q&A from one of our 11 Plank Mentors and discuss its application with your team. Everything you’ll need to host your event is included in our T2G Talk & Taste kit.

Send us a picture of your event with a completed reimbursement form and your team meal will be on us (up to $100). If you can’t hold your T2G Talk & Taste on Nov. 7, then aim for any other day that week and it will still count! It’s not too late to plan your event: RSVP today!

Plank Mentor of the Month: Valerie Spier, M.P.H., RD, CDE

Campaign Plank: Refer to Diabetes Self-Management Education and Support Programs

Valerie Spier has 17 years of direct patient care experience as a diabetes educator at the Palo Alto Medical Foundation. In 2017, she joined the Sutter Health Quality and Clinical Effectiveness Team (1/2 time) as a consultant which includes the role of SHIDEN (Sutter Health Integrated Diabetes Education Network) quality coordinator.

Campaign Spotlight

We are thrilled to share that the case studies from the T2G Innovator Track Cardiovascular Disease (CVD) Cohort are now available! From May 2018 to May 2019, the 12 medical groups and health systems that participated in this Cohort explored how providers can better identify and manage CVD risk among their patients with Type 2 diabetes. Over the course of this initiative, participants saw improvements in all six Cohort measures using a variety of interventions. In the case studies, organizations provide detailed insight into their interventions, outcomes, and lessons learned.

Resource of the Month

This month, we are highlighting the CLOT WISE TM education program from Janssen Pharmaceuticals, Inc.—the Presenting Corporate Collaborator of the T2G campaign extension. The CLOT WISETM program includes videos to help you learn more about the underlying risk for blood clots related to coronary artery disease (CAD) and peripheral artery disease (PAD), why aspirin alone may not be enough, and how to further reduce your patients’ risk for cardiovascular events.
