. This premier gathering of leaders from medical groups and health systems provides the perfect venue to network, discover shared learnings, and exchange strategies for success with your peers. T2G highlights include:
- The breakout session, “Achieving and Sustaining Improved Cardiovascular Risk Care for Diabetes Patients: Building Lessons of the Together 2 Goal® Innovator Track CVD Cohort.” This breakout session will feature best practices gleaned during the Cohort from Geisinger, Sharp Rees-Stealy Medical Centers, and Southwest Medical Associates.
- The AMGA Foundation Celebration Luncheon taking place March 28, 2020 at 12:00 p.m. PST. This event will honor a number of award winners across the Foundation, celebrate the campaign milestone of 1 million lives improved, and serve as a networking opportunity for organizations involved in Foundation activities. All conference attendees are encouraged to attend.
