The Million Hearts^® Hypertension Control Challenge is back! The 2020 Challenge is open to group practices and health systems that achieved hypertension control rates of at least 80% for a 12-month period. If your efforts through Together 2 Goal^® have successfully reduced high blood pressure in your patients, share your success story and you could join other campaign participants that have been recognized in these annual awards celebrating exceptional hypertension control.
