Medical groups and health systems can join peers across the nation in our collective effort to improve care for people with Type 2 diabetes. Participation in Together 2 Goal® is complimentary for AMGA members. Non-AMGA members can pay a nominal campaign fee or apply for hardship consideration.

Ready to join? Complete the brief campaign enrollment form.

The form requests four campaign points of contact within your organization, each with a specific role as described below. Depending on the size and structure of your organization, one person may serve multiple roles. Following completion of the campaign enrollment form, we will send a welcome email to all team members.

  • Primary contact: manages and oversees all campaign activities.

  • Quality contact: leads campaign plank implementation and other relevant activities at your organization.

  • Data/IT contact: oversees the campaign quarterly data reporting process at your organization. This individual will receive information about data reporting deadlines but does not need to be the individual who will submit data via the data portal or Excel template.

  • Marketing/PR contact: responsible for internal and/or external communications about the Together 2 Goal® campaign on behalf of your organization. We will communicate with this individual about logo usage, media opportunities, award recognition, and more.
